طب يا جماعه بلان بى مش بطاله اهو ممكن نحاول نذاكر الكمبرهنسف (لعنة الله عليه) و معاه ايلتس اكاديمك(مهو احنا نطلع من نقرة الفرنساوى و نقع فى دحديرة الايلتس :ninja: )

انا كمان دعبست على بلان سى كمان و هى ال Pharmacy Technician ده غير ال Pharmacy Assistant و دى برضه مش بطاله اهى على الأقل احسن من شغل الداتا انترى او السكيوريتى و دى ليها معادله لوحدها بس اعتقد انها اسهل بس بصراحه مش متأكد لو فيه حد عنده معلومات عن الموضوع ده ياريت يفيدنا ... انا عن نفسى بعت ايميل للبورد بتاع ال Technicians عشان مش فاهم حاجه من الموقع و لو ردو عليا هبقا اقولكم قالو ايه

حلوه قبيلة الاناكوندا دى يا دكتور حسام انا شخصيا متبرع انى انقل ملخص اللى اتكتب عن خطوات المعادله و هضيف عليها المعلومات اللى عندى و بالنسبه لتوسيع المشاركات انا نفسى بجد نعمل كده و اللى عنده اى فكره يقولها حتى لو شايف انها فكره مش عمليه اهو الكلام بيجيب بعضه و ممكن حاجه مش مهمه عند واحد تبقى مهمه عند واحد تانى .

تحياتى للأخوه و الأخوات الصيادله،

المشاركه دى عباره عن تجميع كل اللى اتقال عن معادلة الصيدله فى كندا سواء فى كيبك او الولايات التانيه و ده اقتباس من مشاركة الدكتور Mithras صاحب التوبيك ده انا مكتبش حاجه من عندى ماعدا بعض المعلومات الجديده انا هعملها أبديت بعد اذن الدكتور Mithras وكمان فيه معلومات منقوله من مشاركات للدكتور بات مان و لناس تانيين فى المنتدى هنا و منتديات تانيه

كبدايه، شهادة معادله الصيدلة هي أهم و ربما أصعب خطوه في سبيل الحصول على رخصه مزاوله الصيدلة في كندا. و بدون تلك الشهادة لا يمكن الحصول على ترخيص مزاوله المهنة في أي مقاطعه من مقاطعات كندا باستثناء كيبيك لوجود نظام معادله خاص بها. فإذا كنت تريد الاستقرار في كيبيك فتلك الشهادة لن تفيدك فى العمل هناك. أما لبقية كندا فهي عين المطلوب[/u](بس دلوقتى الوضع ده اتغير و بالنسبه لكيبك برضه بقت الشهاده دى بتمشى بس تعدى امتحان لغه فرنسيه ).

الجهة المختصة بتقييم شهاداتك و عقد الامتحانات هيPharmacy Examining Board Of Canada و ليست أي جهة أخرى.

الخطوة الأولى هي تقييم الوثائق، و هي كالآتي

1- ملى طلب لتقييم شهاداتك و توثيقه (يمكن طلبه بالبريد أو طبعه من الموقع) Application for document evaluation

2- مبلغ 500 دولار كندى يرسل كشيك مصرفى ل PEBC

3- وثائق إثبات هوية وهى

• شهادة الميلاد مترجمه (تترجم في مكتب الطوبجى)(دلوفتى بتروح تترجمها علطول فى وزارة الصحه قدام مجلس الشعب ب 40 جنيه هيعملو كل حاجه)

• و شهادة الزواج مترجمه لو كنت متزوج(دى بتترجم فى وزارة العدل قريبه من وزارة الصحه)

4- بالنسبة لمن أصبح مقيم مستديم إثبات حالته.

5- وثائق إثبات التخرج و الترخيص و هي :

• نسخه لشهادة التخرج الأصلية (الكرتون المعلقة على الحائط) موثقه، بالاضافه إلى ترجمه رسميه (من مكتب فؤاد نعمه بجوار الحرية مول)، هذه الشهادة يرسلها المتقدم مباشره ل PEBC

• شهادة بيان درجات بالدرجات و التقديرات و ترجمتها إن لزم الأمر، ترسلها الجامعة مباشره لPEBC

• شهادة حسن سير و سلوك من نقابه صيادلة مصر باللغة الانجليزيه، حديث لم يمر عليه أكثر من 6 شهور و يرسل من النقابة مباشره.

تفاصيل التقديم على امتحان البورد الكندي

1- يجب أن تعلم أن هناك أربع طرود منفصلين لازم يوصلوا مقر البورد الكندي في تورنتو (العنوان بتاعهم موجود في الموقع اللي أنا كتبه فوق

2- الظرف الأول يحتوى على بيان تقديرات عربي وانجليزي transcription من كليه الصيدلة اللي سيادتك متخرج منها (مع أنى شاكك انك صيدلي) وتصدقه من أمانه الجامعة وترجع الكلية وعند أمين الكلية تجيب ظرف مختوم وهوة يقفله ويختمه.

الظرف ده لما تروح DHL تكتب الاتى:

Sender name: Faculty of Pharmacy Cairo University.

Receiver name the pharmacy examining board of Canada

وتكتب طبعا عنوان البورد اللي على الموقع.

3- الظرف التانى تاخد ترخيص مزاوله المهنة العربي بتاعك وتروح بيه وزاره الصحة اللي في لازوغلى وتطلع جواب يثبت انك لسه صيدلي مرخص ( هما عارفين بس قللهم أنا بعمل معادله).

الظرف ده لما تروح تبعته حيكون زى الأول بس عنوان الرا سل يكون

Ministry of Health

4- الظرف التالت تروح النقابة وتطلع شهادة حسن سير و سلوك باللغة الانجليزية وتجيب برضه ظرف مختوم وتبعته برضه منفصل وزيهم بالضبط بس عنوان لراسل

Syndicate of pharmacy.

5- الظرف الرابع وده ممل شويه

شهاده ميلاد عربي كومبيوتر و تترجمها فى وزارة الصحه

- شهاده الكارتون بتاعه التخرج العربي تترجمها في مكتب ترجمه معتمد من كندا (كان في اللي قدام الحرية مول بالضبط أمام الباب الرئيسي) اسمه اعتقد فؤاد نعمت

- تعمل صور من شهاده الميلاد العربى والانجليزي وشهادة الكارتون العربي والانجايزى مع الاثنين application

حقولك عليهم دلوقتى وتروح بيهم مع 4 صورلسفاره كندا فئ جاردن سيتي وتعملهم notilization ومعاك 900 جنيه .

-application دول تنزلهم من على الموقع وتملاهم كويس جدان وتلزق بجلو صوره زى الباسبور حديثه طبعان على كل طلب في المكان المخصص وتسيب صورتين مرميين في الظرف اللي حتبعته على نفس العنوان بس المرسل المرة دى اسمك انت

-- خلى بالك كل ظرف لوحده في كيس منفصل.

-- تروح بنك نوفا سكوتيه اللي جنب سفارة إسرائيل (وممكن فى البنك الاهلى كمان بس بشرط يكون ليك حساب عندهم) وتعمل اثنين bank draft واحد ب515 واحد ب500 دولار كندى وتكتب اسم المستفيد

The pharmacy examining board of Canada و ترفيقهم مع الظرف الرابع خلى بالك شرط أنا لمحته جديد على موقع البورد أنهم زوده الفلوس شويه يا ريت تتأكد من المبلغ الصحيح علشان الورق ميرجعلك---

---- في الapplication لازم يكون اسمك واسمها زى الباسبور بالضبط مفيش اختلاف.

اي حد ممكن يشتغلها مش بس الصيادله الي فالمعادله لا كتير طلبه فالجامعات بيشتغلوها وخدو دي كمان في كيبك دلوقتي عشان تشتغل مساعد لازملك دراسه فمعهد خاص كده ولا زم طبعا الفرنساوي فله ليييه؟

مهو في شباب كيبكوا بيدرسوا فالجامعه وبيصرفوا على نفسهم بشغلانه الاسيستانس

متأكد من معلومة ان الصيدلي ماينفعشي يشتغل أسيستانت من الأول حالص قبل أى جزء فى المعادلة وفى كل الأقاليم الكندية؟

انا كنت سمعت من فتره انك عشان تشتغل اسيستنت لازم يكون معاك بارت 1(افالويشن) من المعادله

وسمعت ان الاطباء كمان بيزاحمو الصيادله على موضوع الاسيستنت دى كمان....تخيل؟

يا باشا مفيش علاقه انك تشتغل اسيستنس والمعادله دي سكه ودي سكه

الانترنشيب هوا الي له علاقه بموضوع البورد والمعادله

و تسجيل حيرة في نفس الوقت من الوضع الغامض هناااك دة

يعني بالرغم ان الصيادلة يعتبرو

اكتر فئة مهاجرة

واكبر عدد على صفحات منتديات الهجرة

ومع ذلك مش قادرين نحدد بالظبط

ايه الواحد ممكن يعمله لحد مايخد الترخيص

There are two separate application forms and two separate fees for Pharmacist Document Evaluation and The Pharmacist Evaluating Examination. The fee for Pharmacist Document Evaluation is CAD$530. The fee for the Pharmacist Evaluating Examination is CAD$515.

Incorrectly submitted documents will delay the evaluation of your qualification.

If you have any questions regarding documents required, please contact our office.

Deadline Dates

You may send your documents to us anytime because evaluations are completed all year. However, please note: if you wish to be considered for the next Evaluating Examination sitting, all documents, applications and fees must be correctly submitted by the deadline date.

Deadline Dates:

October 9 in order to apply for the January 13 & 14, 2010 Pharmacist Evaluating Examination.

April 9 in order to apply for the July 14 & 15, 2010 Pharmacist Evaluating Examination.

Please note: If you are planning to apply for the next Evaluating Examination, all documents, applications and fees (including Document Evaluation Application form, fee and documents and Evaluating Examination Application form and fee) must be correctly submitted by the deadline date.

There are two separate application forms and two separate fees for the Pharmacist Document Evaluation and The Pharmacist Evaluating Examination. The fee for The Pharmacists Document Evaluation is CAD$530. The fee for the Pharmacist Evaluating Examination is CAD$515.

Please note: There is an additional administration fee of CAD$365 for the London, England site of the Evaluating Examination.The application form and fee for the Evaluating Examination can be sent at the same time as the application form and fee for Document Evaluation.

Authorization

If you wish someone to act on your behalf, please fill out, sign and send the Authorization Statement, Form 200. Please note: We need your original signature on this Form so it must be sent by mail.

Please fill out all areas. Your signature and photograph on this form must be witnessed by a notary public, commissioner for oaths, lawyer or the Canadian Embassy. Please send two passport-sized and quality photographs (taken within the last year). One photograph should be attached in the space provided on the Application Form, the other can be stapled to the top left hand corner.

We do not accept faxed or e-mailed applications.

If you want to know if your application has been received, please send Form 100-D with a self-addressed, stamped envelope (within Canada) or a self-addressed envelope with International Reply coupons (outside of Canada) and the and the form will be returned to you by mail. If the form is received without the appropriate return envelope and postage, it will not be acknowledged.

Your application will be acknowledged via email or regular mail as quickly as possible after it is received

CAD$530 Document Evaluation Fee

Please submit the fee in Canadian currency payable to "The Pharmacy Examining Board of Canada" or "PEBC". We do not accept currency other than Canadian dollars. All payments made outside Canada must be made in Canadian funds by international bank draft or money order drawn on a Canadian Bank.

Payments within Canada may be made by personal cheque if you wish.

Cash payments will not be accepted. If your payment is returned by our bank, you will be charged an additional $50.

Documents to Support Identity

Birth Certificate certified as a true copy of the original by a notary public, commissioner for oaths, lawyer or the Canadian Embassy with attached official translation (if necessary). A "Birth Certificate" issued by a Consulate will not be accepted. OR

Canadian Citizenship Card (both sides) certified as a true copy of the original by a notary public, commissioner for oaths, lawyer or the Canadian Embassy. (We will accept a Canadian Citizenship Card instead of a birth certificate).

Please Note: If you do not have an official document called a birth certificate or a Canadian Citizenship Card, you, personally, must make a statutory declaration before a notary public, commissioner for oaths or lawyer stating that the name shown on your application is that given to you at birth, your date of birth and the reason why you don't have a birth certificate. You must also submit certified copies of two official documents bearing that name and birth date in support of the declaration. Acceptable documents include a baptismal certificate, social insurance card, passport, driver's licence, secondary school certificate, Immigration Record of Landing or current Permanent Resident Card, etc. Please ensure that the names on your supporting documents are spelled exactly the same as in your declaration.

Marriage Certificate certified as a true copy of the original by a notary public, commissioner for oaths, lawyer or the Canadian Embassy with attached official translation (if necessary). This document is only necessary if you have changed your name after marriage and wish to use your married name.

Please Note: If the name on your Application Form is different from the one on your birth and marriage certificates, you must make a statutory declaration before a notary public, commissioner for oaths, lawyer or the Canadian Embassy stating:

1. The name by which you wish to be known;

2. That the names shown on your various documents refer to the same person;

3. An explanation for the differences in the names.

We also require certified copies of two official documents bearing the name you wish to use.

If you have legally changed your name, please submit a certified copy of your legal change of name document.

Immigration Document

When you arrive in Canada as a landed immigrant, you will receive an Immigration Record of Landing Certificate or Permanent Resident Card. At that time, please submit certified copies of both sides of this document. If you have not yet arrived in Canada, we will proceed with the evaluation of your qualification without this document.

Documents to Support Graduation and Licensing

University Degree Certificate Please submit a copy (certified as a true copy of the original by a notary public, commissioner for oaths, lawyer or the Canadian Embassy) of your original language University Degree Certificate (the large one that you would frame and hang on the wall) with an attached official translation. Please note that this document must be submitted by the candidate.

Transcript We require an originally produced, currently-dated transcript (with an attached official translation, if necessary) mailed directly to our office by the University. Attested copies will not be accepted. This document should include marks or grades obtained for each subject in each year.

If the University is not able to provide us with this document, please contact the office for instructions.

Licensing Statement Please notify the Licensing Authority in the country where you are currently licensed, to mail a currently-dated letter directly to our office, stating that you are currently licensed as a pharmacist and your license is in good standing (i.e., there are currently no disciplinary proceedings or actions against you). A letter of good standing which is not mailed by the licensing authority will not be accepted by PEBC. Please note: This letter is only valid for a period of six months from the date it was issued and must be received in our office within 1 month of the issue date (see "Common Mistakes" (below).

If you are not currently licensed as a pharmacist please contact the office for instructions.

Important Information

Translations

If documents are in a language other than English or French, they must be translated by a Government Official or an official translator. Do not prepare your own translation.

Translations must be attached to the document they are translated from.

Certified Documents

Where certified copies are requested they must be certified as true copies of the original by a notary public, commissioner for oaths, lawyer or the Canadian Embassy who writes the phrase "This is a true copy of the original document" and then signs and stamps it.

If the certification is done in a language other than English or French, please send an official translation of the certifier's stamp, etc.

Photographs

Please send passport-sized and quality photographs which look like you and are taken within the last year. One photograph should be attached to the application form in the space provided. The other can be placed at the top left hand corner and should have the date it was taken stamped on the back.

Document Evaluation Result Letter

This letter will be mailed to you approximately 4-8 weeks after we have received all the necessary documents, applications and fees, correctly submitted.
